An open bite is easy to recognise: when the jaws are closed, a visible gap remains between the upper and lower teeth. Most often it appears in the smile zone and makes biting difficult; less often in the side segments, where the molars do not meet. The lower third of the face lengthens, the lips do not close and the mouth stays slightly open.

In children the causes are prolonged thumb or dummy sucking, infantile swallowing with the tongue pushing between the teeth, and mouth breathing with adenoids. In adults an open bite may be caused by endocrine disorders, osteoporosis, severe periodontitis and injuries. The anomaly is often combined with incisor crowding, enamel wear and joint dysfunction.

Untreated, speech suffers — especially sibilant sounds — as do digestion and the joint, and the mouth dries out. At iOrtho in Yerevan an open bite is corrected with aligners and braces with inter-jaw elastics; for a pronounced skeletal form orthodontics becomes the preparation stage before a surgeon's consultation. In children treatment is complemented by myofunctional exercises.

Benefits

  • An interdisciplinary approach

For mouth breathing we refer to an ENT, for joint problems to a gnathologist, so the result holds.

  • A virtual setup

With aligners treatment is modelled step by step: you see every tooth movement and the final result.

  • Children — from an early age

Aligners and myofunctional exercises teach the tongue its correct position and turn infantile swallowing into adult swallowing.

  • Health, not just a smile

Closing the gap restores nasal breathing and normal speech and relieves the joint.

Indications

  • A gap between the front teeth with the jaws closed
  • The side teeth do not meet on one or both sides
  • The tongue shows between the teeth when smiling or talking
  • Lisping, difficulty biting an apple or bread
  • An elongated face, lips do not close, mouth slightly open
  • Dry mouth, snoring, headaches from joint overload

Treatment stages

  1. 1.

    Consultation

History, assessment of swallowing, breathing, speech and the joint, a preliminary plan.

  2. 2.

    CT and scanning

Low-dose CT shows the skeletal component; the scanner builds a 3D model of the arches.

  3. 3.

    Preparation and related specialists

Professional hygiene, treatment of caries and gums; an ENT and gnathologist if needed.

  4. 4.

    Fitting the system

The first aligners with attachments, or bonding braces and inter-jaw elastics that close the vertical gap.

  5. 5.

    Monitoring

With aligners every 2–4 months, with braces every 1–1.5 months. Children do myofunctional exercises.

  6. 6.

    Retention

Fixed retainers and night trays. Duration — twice the active treatment time, or lifelong.

Care rules

  • Tongue on the palate

Learn to keep the tongue in the front third of the palate and swallow without pushing it between the teeth — otherwise the gap reopens.

  • Bad habits

Children — wean off the dummy, thumb and lip biting. Adults — no biting pens or nails.

  • Elastics and trays

Wear inter-jaw elastics as instructed and aligners 20–22 hours a day.

Questions and answers

Can an open bite be fixed without surgery?

Yes, in most cases — with aligners or braces with elastics. Surgery is discussed only for a severe skeletal form.

How long does treatment take?

It depends on complexity and age: faster in teenagers, on average 2–3 years in adults because bone remodels more slowly.

At what age should a child start treatment?

At the first signs. In the milk dentition myofunctional exercises, trainers and children's aligners work; braces usually from 12–14.

Can an open bite cause headaches?

Yes — through constant muscle tension and overload of the temporomandibular joint. After correction the load is distributed evenly.
