Consent to Personal Data Processing

By submitting the consultation booking, callback or review form on the website, I, the data subject, freely, of my own will and in my own interest give the orthodontic clinic iOrtho Yerevan (Yerevan, Republic of Armenia; the "Clinic") my consent to process my personal data on the terms below, in accordance with the Law of the Republic of Armenia "On Protection of Personal Data".

1. Data covered

  • name;
  • phone number;
  • e-mail address (if given);
  • preferred contact channel (call, WhatsApp, Viber);
  • message or review text, rating and photo (review form, if I added them);
  • the page of the website the form was sent from.

2. Purposes

  • contacting me to book a consultation and confirm the appointment time;
  • calling me back and answering my question;
  • publishing my review on the Clinic's website (review form only);
  • keeping records of requests to the Clinic.

3. Processing operations

Collection, recording, organisation, storage, updating, use, transfer to the messenger I chose for contact, anonymisation, blocking, erasure and destruction — with or without automated means.

4. Duration

The consent is valid for 12 months from the date the form is submitted or, if I become a patient of the Clinic, for the medical-records retention period set by the legislation of the Republic of Armenia.

5. Withdrawal

I may withdraw this consent at any time by e-mail to . On receipt the Clinic stops processing and deletes the data unless the law requires otherwise. Withdrawal does not affect the lawfulness of processing carried out before it.

6. Confirmations

  • I am over 18, or I am the parent or legal representative of the child I am booking for and I provide my own contact details;
  • I have read the Clinic's Privacy Policy and know my rights as a data subject: to be informed about the processing, to have data corrected, deleted or blocked, to withdraw consent and to complain about the Clinic's actions to the authorised personal data protection body of the Republic of Armenia or to a court;
  • the data I provided is accurate.
