When to Take a Child to the Orthodontist for the First Time

3 min read

Many parents believe a child should see an orthodontist only after all the permanent teeth have come in. In fact, the first visit should be planned much earlier, at a stage when jaw growth can still be guided with gentle methods.

At what age to take a child to the orthodontist

A routine first check-up is recommended at 5–7 years of age, before the primary teeth are replaced by permanent ones. At this age, the doctor assesses the proportional growth of the jaws, the developing bite, the space available for permanent teeth, as well as tongue position, swallowing pattern, and breathing type.

It is best not to postpone the visit beyond age 7. Early diagnosis makes it possible to notice the first signs of problems and correct them more easily than in adolescence, when jaw growth is nearly complete. The first check-up does not mean treatment starts; it is a preventive assessment.

  • No complaints, child aged 5–7 — a routine first check-up.
  • Under 5 but with warning signs — come in without waiting for the routine age.
  • Teeth have started to change — a consultation to make sure the permanent teeth are erupting correctly.
  • Teenager aged 12–14 who has never seen an orthodontist — a consultation soon to check the permanent bite.

Signs that should alert parents

Visible signs

  • facial asymmetry or a shifted chin;
  • crowded teeth or large gaps between them;
  • disproportionate jaws, with the upper or lower jaw protruding noticeably;
  • a deep overbite, where the lower teeth injure the palate.

Breathing, sleep, speech, chewing

  • regular mouth breathing, especially during sleep, or snoring with no ENT disease;
  • difficulty chewing, biting the cheek or tongue;
  • speech problems that a speech therapist cannot resolve within 2–3 months.

Habits and oral condition

  • thumb sucking or pacifier use after age 2;
  • early loss of primary teeth due to injury or decay;
  • noticeable lip strain when swallowing.

What happens at the first appointment

No special preparation is needed. An iOrtho orthodontist first talks with the parent to learn about the child's development, the age when the first teeth appeared, and past illnesses. Then comes the examination: the doctor assesses how the jaws close, the condition of the teeth and gums, facial symmetry, lower-jaw movement, the attachment of the tongue and lip frenula, and the type of breathing and swallowing.

A CT scan is usually not needed at the first visit; it is prescribed individually for delayed eruption, injuries, or before major treatment. If a decision is made to correct the bite, a photo protocol is taken to record the starting condition and track progress.

What the orthodontist may prescribe

  1. Observation. If jaw growth and eruption are normal, no treatment is needed; the doctor sets up a schedule of follow-up check-ups.
  2. Correction of habits and functions. For mouth breathing, a consultation with an ENT specialist; for speech issues, a speech therapist; myofunctional exercises when indicated.
  3. Removable appliances. Between ages 3 and 10, plates, trainers, and activators are used to train the muscles and guide jaw growth.
  4. Braces or aligners. Only once the permanent bite has formed, usually from age 12–14. In some cases, earlier treatment with children's aligners is possible.

Frequently asked questions from parents

Is a consultation needed if the teeth look straight?

Yes. A visually straight row does not rule out hidden problems related to underdeveloped jaws, joint dysfunction, or incorrect breathing and swallowing.

Do crooked teeth always require treatment?

No. In the primary and mixed dentition, gaps between teeth are often normal: they indicate jaw growth and preparation of space for permanent teeth. A gap between the upper incisors before the canines erupt is physiological and usually closes by age 12–13.

Will it hurt the child?

The examination is completely painless. The doctor uses no instruments and makes no sudden movements; the appointment takes place in a calm setting.
